Manager San Diego Youth Services seeks a Social Work Case Manager
in the Resource Family Agency (RFA) to be based in San Diego. This
is a full-time & exempt position reporting to the Program Manager.

vulnerable youth since its beginning in 1970. We provide safe
places to live and long-term solutions through shelter, foster
homes, community centers and housing to more than 28,000 youth
every year at 100 community and school locations. Through
prevention, early intervention and treatment, we also help youth
before they need higher levels of care or become homeless. We aim

has long provided foster homes and adoption support services for
families considering adopting a youth in the foster care system.
Help youth develop life skills. In addition to a range of safe and
stable housing options, we offer support to help youth develop the
independent living skills necessary to become self-sufficient and
keep off the streets. Promote overall health and well-being. Early
intervention and care for the whole child are critical. We provide
a broad continuum of services to help youth overcome trauma and
challenges that put them at risk, including case management,
counseling, mental health treatment and educational support and
resources. SDYS has a staff of approximately 250 employees and 80
volunteers working across San Diego County and an annual budget of

www.sdyouthservices.org. PROGRAM DESCRIPTION RFA provides safe,
structured, and caring home environments for abused and neglected
children (under the age of 18). Our foster youth are given a second
chance to form healthy relationships and develop the necessary
skills to successfully transition to adulthood. RFA provides
specialized services for foster youth such as respite assistance,
24-hour support, comprehensive training, counseling, and
psychiatric services. CONTEXT OF POSITION Accountable to the
Program Manager. Responsible for the placement and supportive care
of children/youth in certified resource family agency homes.
Responsible for the management of a caseload including development,
documentation, implementation, and evaluation of client plans,
supervision and reporting of the same. ESSENTIAL FUNCTIONS:
Participate in the matching, transition, and placement of youth in
foster homes. Responsible for implementation, compliance, and
documentation of Community Care Licensing regulations, San Diego
County Contract, and SDYS Policies and Procedures and participation
in regular meetings. Provide assessment, risk assessment, crisis
intervention, Client plan development, progress notes, quarterly
reports, incident reports and case management which includes
documentation in client EHR in a timely manner. Advise and support
foster parents on compliance of CCL regulations. Monitor the foster
family homes to ensure that they are in compliance with governing
rules & regulations. Maintain professional relationships and
contact with placement workers, case managers, therapists,
teachers, advocates, attorneys, psychiatrists, doctors, dentists
and other child welfare professionals. Respond to service partners
during a crisis or urgent matter, including those that occur
outside of normal business operating hours, by implementing and
following the on-call procedures whenever the rotation of on-call
falls into the scope of responsibilities for the assigned period.
Perform in home visitations weekly or as needed. Perform other
related duties as assigned. SDYS EXPECTATIONS: Adhere to and uphold

confidentiality and privacy standards of service partner protected
health information and other applicable information and material in
accordance to agency and center policies, and applicable
regulations and laws including HIPAA. Welcome and support SDYS
volunteers and assist them in accomplishing their services whenever
applicable. Effectively communicate with people from diverse
backgrounds and experiences. Recognize, nurture, and learn from
strengths in co-workers. Maintain warm, welcoming and safe work
environments. Possess flexibility to accomplish job
responsibilities in several work environments, whether it be from
home, on the road, or in an office. Employment is subject to
required clearances which include tuberculosis test,
fingerprinting, background checks, and drug testing. REQUIREMENTS
AND QUALIFICATIONS: Master's degree in social work, marriage and
family therapy, or related field, and field experience/practice in
public or private social service agency at a master’s degree level,
18 months experience working with children and families and two
years’ experience in public or private child welfare services
setting. Semester units and/or related coursework can be considered
in place of experience. Must provide transcripts. Familiarity with
child welfare system, youth who have experienced trauma, and case
management experience preferred. Must be able to work within and
represent a culturally and ethnically diverse community. Must
complete clearances including child abuse/criminal background
check, health and substance abuse screening. Must have a reliable
vehicle, auto insurance, and good driving record. Must be willing
to develop or possess working knowledge of trauma-informed care and
dual diagnosis treatment; experience working in trauma-informed and
dual diagnosis programs preferred. Must be able to obtain/maintain
CPR and First Aid Certification. PHYSICAL REQUIREMENTS: The ability
to bend at the waist, kneel, or crouch to assist youth. The ability
to communicate information and ideas so others will understand, to
exchange accurate information and make presentations. The ability
to lift and carry 20 pounds-lifting books, materials, and equipment
to execute lessons and presentations. Able to observe details at a
close range and interpret the information (such as reading),
prepare, and review a variety of documents related to activities of
the youth and to be able to monitor youth activities. Sitting or
standing (stationary position) for extended periods of time.
Traversing extended lengths to move around campuses. Potential
exposure to physical injury from aggressive behavior. Salary Range:
